What is the Impact of a Local Model of Collaboration on Access to and Quality of Multi-disciplinary Care for Patients with Chronic and Complex Health Problems?

This project aims to identify, develop and test the systems and structures required to support more effective and accessible multi-disciplinary team care for patients with chronic and complex health problems, through promotion of collaborative organisation, as well as linkages within the primary health care sector.
