Aboriginal and Torres Strait Islander chronic disease management models

Aboriginal and Torres Strait Islander peoples experience markedly elevated and premature CVD risk. Our forthcoming work indicates high absolute CVD risk at very early ages in the Indigenous population and that current assessment guidelines require revision for optimal CVD risk assessment in this population. Future work will focus on quantification of absolute CVD risk, guideline revision and implementation by increasing best-practice absolute CVD assessment and management through primary care.
